Long Term Home Health Program

Purpose: 
   
To address the patient's safety, physiological and psychological needs in a patient's plan of care.

    To address comprehensive quality patient and family centered care in the patient's place of residence.

    To provide home health care services that promote the patient's value of life by providing those
    services in the most appropriate home care program.

The long-term home health program (LTHHCP) and its services have been available in Washington County since 1985.  This is a New York Department of Health certified program to provide a multidisciplinary array of services to residents who qualify for nursing home placement, but can and prefer to stay at home.  The program is primarily funded by Medicaid but can accept private pay patients.  The LTHHCP is a jointly run program with Washington County Department of Social Services.  The focus of the program is to provide a cost effective, comprehensive alternative to nursing home placement and remain within 85% of nursing home costs.
